The School Health Liaison aligns the services from our community health center with area schools, ensuring that students and families can easily access the care they need. Work closely with schools and families to coordinate services, address barriers, and create a seamless support network for children.
Qualifications
- Bachelor’s degree in Public Health, Health Administration, Nursing, Social Work, Education, or related field.
- 1-3 years of program management experience in healthcare, public health, or community-based programs.
- Experience working with low-income families in community or school settings.
- Experience with assisting in the coordination and implementation of program activities for the community.

Key Accountabilities
- Serve as the primary link between area school districts to manage access to our medical, behavioral health, insurance enrollment and dental care services (Seal-A-Smile program).
- Coordinate referrals, appointments and follow-up care for students, ensuring timely access to services.
- Facilitate communication between providers, parents/guardians, and school personnel while maintaining confidentiality and compliance with HIPAA.
- Identify and address barriers to care, such as transportation, insurance, or language access, connecting families to community resources.
- Oversee data collection systems to ensure accuracy, completeness, and compliance with grant requirements.
- Monitor key performance indicators including student utilization, access, referral completion, and outcomes. Develop strategies to increase student enrollment and program utilization.
- Organize on-site health events such as dental screenings, immunization clinics, and mental health awareness programs.
- Support uninsured students and families in completing a sliding fee scale (SFS) or hardship application. Refer students and families to our Enrollment team for health insurance enrollment assistance.
- Attend planning committee meetings for school districts to establish a direct referral relationship and process.
- Regularly participate in onsite events to promote PCHC services that reach parents, including: school conferences, annual registration events and related programs.
- Complete monthly report on program outcomes.
- Perform other duties as assigned.
Compensation
24.34-33.10 per hour
